Qualifications:

Education:

Bachelor's degree in social work or related experience and/or education equivalent preferred. Ohio license (LSW, LPC) preferred. Minimum of AA degree in a psychology or related human services field and experience in human services with continued pursuit to obtain a Bachelor's degree.

Experience:

Knowledge of mental illness and recovery; experience in a mental health or social service setting; good interpersonal, verbal and written communication skills essential.

General:

Must meet satisfactory criteria of the agency with regard to required Bureau of Motor Vehicles driving record check, proof of valid driver's license and criminal background check; must have reliable transportation and automobile insurance.

Responsibilities:

  • Meets or exceed agency direct service and client service standards and goals. Maintains complete and timely documentation in accordance with all applicable agency standards.
  • Conduct ongoing assessment of client's mental health and related needs for improving community living skills.
  • Works collaboratively with clients to develop individual service plans that address client needs, incorporate client preferences for services, and to monitor the implementation of the plan and progress toward goals.
  • Provides direct service support to clients in their homes and communities, as well as in agency offices, to promote client access to appropriate services. Provides support to clients and families in crisis.
  • Is knowledgeable of, and makes referrals to appropriate service systems, and serves as a client advocate in navigating various health, education, housing and social service systems to obtain necessary supports.
  • Provides education and support to clients and families re: mental illness, relapse prevention, avenues to recovery, and available community supports and self-help services.
  • Interacts in a professional and respectful manner at all times with clients and families, agency co-workers, and other community agency staff.
